You’ll need to call 1-888-746-7726, choose option #2 and then tell the representative you are calling for the New Mom Membership offer (note that I was on hold for over 15 minutes when calling about this offer, so you will need a lot of patience!).
After providing your full name and mailing address, you will be given a membership number – you will need to take this number to the customer service desk at your local Sam’s Club where you will be issued a physical membership card.
